Original and photocopy of the vehicle owner's identity proof. 3. Original "Motor Vehicle Registration Certificate." 4. Original "Motor Vehicle License." 5. "Motor Vehicle Inspection Record Form" issued by the inspecting officer after confirming the vehicle.
