You can obtain the "Request for Access to Record of Private Body" form (Annexure A) from Digital Matter's Information Officer or Deputy Information Officer, the Information Regulator website (www.inforegulator.org.za), or the Department of Justice and Constitutional Development website (www.doj.gov.za). Complete the form with your details, including the specific record you seek and the reason for your request. Note that non-personal requesters typically pay a R50.00 fee. Submit the form and any required fees to Digital Matter. They will review your request within thirty days and notify you of their decision. If approved, you'll be informed of any access fees based on the format of the record (e.g., photocopy). Access is granted after fee payment. If denied, you have thirty days to seek relief from a competent Court or the Information Regulator.
