Required documents: 'Motor Vehicle Driver's License Application Form'; identity proof of the driver; current driver's license; a medical fitness certificate issued by a county-level or above medical institution, or a regimental-level or above military medical institution. For applicants applying for a special small automatic transmission passenger vehicle for the disabled, a medical fitness certificate issued by a specialized medical institution designated by the provincial health authority is required; two recent passport-sized color photos of the driver (taken within the last 6 months, front-facing with a white background, no uniform, dimensions 32mm×22mm, head width 14mm-16mm, head length 19mm-22mm, resolution 300dpi). 2. Eligibility criteria: If the applicant is applying for both a replacement and renewal of the driver's license, the current driver's license will not be reviewed.
