Where to Obtain the Physical Condition Certificate for Driver's License?

Driver's license physical condition certificates are processed at county-level or higher medical institutions, or military medical institutions at the regimental level or above. Introduction to county-level or higher medical institutions: Municipal-level hospitals and disease control centers confirmed by the Health Bureau; district and county central hospitals within the city. Processing requirements: Bring your ID card, driver's license, and three one-inch color photos to the city's driver's license management office to complete both the physical examination and license renewal procedures; alternatively, you can process at the city's vehicle management office or the local traffic police brigade, where you need to present a physical condition certificate issued by a county-level or higher hospital within the city. According to the Road Traffic Safety Law of the People's Republic of China and the Ministry of Public Security Order No. 71, drivers who are required to undergo an annual physical examination but fail to do so within the year, and still do not submit the physical examination certificate after a one-year grace period, will have their driver's license revoked.
I happened to accompany a friend to handle this matter the other day. You can directly go to a hospital at or above the county level to get it done. I was initially puzzled why it had to be a hospital, but they explained that this type of physical examination must be conducted by a formal medical institution recognized by the traffic management department. Remember to bring your ID card, as some hospitals require advance appointments to avoid long queues. When testing your vision, make sure not to wear contact lenses—the doctor will require an examination with naked eyes. Finally, the red-stamped physical condition certificate must be submitted to the DMV within its validity period. If your vision doesn’t meet the standard or severe hypertension is detected, things could get complicated. It’s advisable to call the hospital’s physical examination department beforehand to avoid a wasted trip.
I encountered this step when renewing my license last year. Now you can check the list of designated local hospitals on the Traffic Management 12123 APP—just pick the nearest one. The physical exam is super simple, mainly checking vision, color blindness, and limb mobility. A little tip: it's best to go in the morning when it's less crowded, and being fasting doesn't affect the check-up. Remember to pay the exam fee at the cashier window with cash or a medical insurance card. The doctor will sign and stamp the certificate on the spot. Most importantly, make sure the hospital is qualified—small roadside clinics' certificates won't be accepted by the DMV. Then just submit the certificate along with your driver's license application form at the counter, and you're done.
