If applying at a vehicle management office outside the original issuing jurisdiction, transfer procedures must be completed as required. The guidelines for applying for a replacement motor vehicle driving license are as follows: 1. Required documents: "Application Form for Motor Vehicle Driving License"; the driver's identity proof; the original driving license; two recent photographs of the driver (specifications: front-facing, full-color, bareheaded, half-length ID photo taken within the last 6 months, white background, no uniform, dimensions 32mm×22mm, head width 14mm-16mm, head length 19mm-22mm, resolution 300dpi). 2. Eligibility criteria: The driver information recorded on the driving license has changed within the jurisdiction of the vehicle management office; the driving license is damaged and illegible; the applicant does not have any of the following conditions: accumulated 12 penalty points, overdue inspection, license under seizure, detention, suspension, cancellation, revocation, or annulment.
