If the information of the motor vehicle driver recorded on the motor vehicle driver's license changes, or if the driver's license is damaged and becomes illegible, the motor vehicle driver must apply for a replacement license at the vehicle management office of the issuing authority or at another vehicle management office outside the issuing jurisdiction within 30 days. If applying at a vehicle management office outside the issuing jurisdiction, the transfer procedures must be completed as required. Guidelines for applying to replace a motor vehicle driver's license: 1. Required documents: 'Motor Vehicle Driver's License Application Form'; the identity proof of the motor vehicle driver; the motor vehicle driver's license; two photos of the motor vehicle driver (the photos must meet the following requirements: taken within the last 6 months before applying for the driver's license service, front-facing, full-color, bareheaded, single-person half-body ID photo with a white background, no uniform clothing, size 32mm×22mm, head width 14mm-16mm, head length 19mm-22mm, resolution 300dpi). 2. Acceptance conditions: within the jurisdiction of the vehicle management office, the motor vehicle driver's license information has changed; the motor vehicle driver's license is damaged and illegible; the applicant does not have any of the following situations: accumulated 12 penalty points, overdue for review, license seized, detained, suspended, canceled, revoked, or rescinded.
