Testing coverage is incomplete—simulated environments can't represent all real-world challenges, such as animals crossing roads. Lastly, human-machine interaction flaws exist—users are unprepared when the system suddenly demands takeover, and over-reliance leads to distracted phone use. These accumulated minor failures can escalate into major accidents, requiring better warning mechanisms and continuous optimization by manufacturers.
