Chinese citizens applying for an international driver's license need to submit the following documents to the International Driving Permit issuing authority: 1. "Motor Vehicle Driver's License Application Form." 2. The applicant's identity proof (original and photocopy). If the documents are not in Chinese, a Chinese translation (original and photocopy) must also be provided. 3. "Motor Vehicle Driver's Physical Condition Certificate" issued by a county-level or higher medical institution or a military medical institution at the regiment level or above. Applicants who are staff members of foreign embassies, consulates, or international organizations in China are exempt from the "Physical Condition Certificate" review. 4. The applicant's current motor vehicle driver's license (must be valid). This should be a foreign driver's license, not an international driver's license. If the license is not in Chinese, a Chinese translation (original and photocopy) must be provided. 5. Six 1-inch color front-facing headshots of the driver without a hat and with a white background.
