Forklift licenses issued before 2011 require an annual review every two years, while those issued after July 2011 are reviewed every four years. Each review process takes four days. The regulations for forklift license reviews are as follows: 1. License holders should apply for a review one month before the license expires. Failure to review or failing the review will result in the license being revoked by the authorities. 2. Review requirements: To apply for a forklift license review, the following materials must be submitted to the quality supervision department: (1) Fill out the "Special Equipment Operator License Application Form." (2) Two photos. (3) A copy of the ID card. (4) The original operation certificate. (5) The training completion certificate must be obtained by the applicant in person.
