especializada.\r\n \r\n Responsabilidades:\r\n Dar la bienvenida a los pacientes a programas de atención continua y revisar los beneficios y servicios incluidos\r\n Coordinar con el Terapista Respiratorio del paciente y los proveedores pulmonares para garantizar un enfoque colaborativo en la atención\r\n Educar a los pacientes sobre la frecuencia y el uso de sus dispositivos de monitoreo en el hogar asignados, si corresponde\r\n Crear un plan de atención personalizado e integral con el paciente mediante llamadas telefónicas o visitas por video\r\n Identificar y abordar cualquier obstáculo que impida el éxito del paciente\r\n Brindar educación específica y orientación sobre las condiciones pulmonares crónicas de los pacientes\r\n Mantener contacto frecuente con el paciente para revisar lecturas y actualizar mensualmente su plan de atención\r\n Actuar como primer contacto del paciente para todas las necesidades no urgentes (por ejemplo, renovación de medicamentos, programación, recordatorios de citas, etc.)\r\n Ayudar a los pacientes a navegar en su atención médica (por ejemplo, ayudar a los pacientes a programar citas, coordinar con especialistas y asegurar que se completen exámenes preventivos, etc.)\r\n Revisar y evaluar en tiempo real las lecturas de los dispositivos en el hogar durante el horario laboral normal\r\n Seguir protocolos establecidos para identificar, comunicar y documentar tendencias del dispositivo y cualquier síntoma asociado en el historial médico del paciente\r\n Elevar lecturas y/o síntomas preocupantes al proveedor siguiendo un salud del paciente\r\n Requisitos\r\n Licencia activa y sin restricciones de Enfermero Registrado en Georgia (se prefiere licencia compacta)\r\n Se prefiere al menos tres (3) años de experiencia en salud adulta\r\n Antecedentes en condiciones crónicas en adultos (Neumología, Cardiología, Manejo de Atención) preferibles\r\n Experiencia variada con registros médicos electrónicos (EMR)\r\n Conocimientos, habilidades y interés competentes en habilidades básicas de computación\r\n Competencia en resolución de problemas y capacidad para realizar múltiples tareas\r\n Excelentes habilidades
