evaluaciones de salud para ajuste de riesgo en una población con condiciones crónicas; nuestras evaluaciones exhaustivas incluyen: una evaluación integral, examen físico, revisión de sistemas corporales, signos vitales, revisión del historial de medicamentos, revisión de recetas actuales, revisión del estado funcional del miembro, manejo del dolor, evaluación cognitiva, revisión del entorno domiciliario, cribado de riesgo de caídas, evaluación social, revisión de actividades de la vida diaria, cribado de salud mental, documentación y comunicación​\r\n Desempeñará un papel
