Responsibilities Conduct in-home evaluations and assessments to determine patients' needs and develop comprehensive treatment plans Implement prescribed physical therapy interventions utilizing effective techniques and modalities Provide education to patients and their families on proper techniques and exercises for promoting mobility and functional independence Monitor patient progress and revise treatment plans as necessary to achieve optimal outcomes Document patient evaluations, treatment plans, and progress notes accurately and timely Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care Adhere to ethical and professional standards, as well as compliance with all regulatory requirements Requirements Valid and active Physical Therapist license in the state of Michigan Prior experience as a PT in a home health care setting is preferred, but not required Excellent communication and interpersonal skills Ability to work independently and effectively manage time and caseload Proficiency in electronic medical records and documentation Compassionate and patient-centered approach to care Valid driver's license and reliable transportation for home visits Benefits 401K Retirement Plan Ability to earn PTO Medical Benefits Available for all 30+ hourly employees Excellent Pay Flexible Schedule!
