This course includes 100 hours of approved instructor lead clinical care of selected adult patients in a long-term care setting.\r\nUpon successful completion of these courses, candidates are eligible to take the State Competency Evaluation for Nursing Assistants. The Street Health Outreach Team supports clients to attain housing stability and improve health and well-being.\r\nUsing a multi-disciplined team approach, the program provides street and venue-based outreach and engagement, assessment, health services and triage, case management, linkages to housing agencies, and mental health services. The program will work with all participants to develop and carry out action plans that improve health and wellness.\r\nUnder the supervision of the Director of Population Health, in collaboration with the Chief Medical Officer, the Street Health Outreach RN Care Manager will complete nursing assessments and provide nursing care including medical assessments, wound care and skin assessments, vaccinations (i.e. flu, Hepatitis A), treatment of respiratory illnesses, and caring for patients with active substance use disorders. This position will supervise the Community Health Worker and the Behavioral Health Case Manager. The RN Care Manager will work with medical providers on behalf of patients.
