Case Manager
Responsibilities Include:
• Conduct assessments to determine client needs and develop individualized service plans.
• Coordinate with community resources to provide clients with access to necessary services.
• Maintain accurate and up-to-date client records and documentation.
• Advocate for clients and assist them in navigating social services systems.
• Facilitate group sessions and workshops to support client development and well-being.
• Collaborate with shelter staff and external agencies to ensure comprehensive care.
• Bachelor's degree in social work, psychology, or a related field preferred.
• Experience working in a homeless shelter or similar environment.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of community resources and social services systems.
