Utilize evidence-based service approaches The Permanent Supportive Housing Case Manager provides comprehensive case management and supportive services to program participants to promote housing stability, self-sufficiency, and overall well-being. Responsibilities include conducting referrals and intakes, developing Individual Service Plans, providing participant orientation, maintaining case files, conducting regular case management meetings and home visits as applicable, and documenting participant progress. The Case Manager assists participants with accessing community resources, benefits, behavioral health and recovery supports, education, employment, and other services that promote long-term stability. The position maintains accurate HMIS records and program documentation and collaborates with internal staff, community partners, employers, educational providers, and other service organizations. This is a floating position that provides coverage and services at two Permanent Supportive Housing locations. The Case Manager must be able to work at both locations and travel between sites as needed. Some evening hours are required to accommodate participant needs, program activities, and service availability. The Case Manager is expected to demonstrate professionalism, sound judgment, cultural responsiveness, confidentiality, strong communication and organizational skills, and the ability to work effectively with individuals from diverse backgrounds. The position participates in required meetings and trainings, serves as a positive role model, and performs other duties as assigned. Minimum Qualifications Bachelor’s degree in social work or a related field preferred, with a minimum of two (2) years of case management experience. Experience providing direct services to individuals experiencing homelessness or other vulnerable populations is strongly preferred. Knowledge of behavioral health, substance use and recovery, harm reduction, motivational interviewing, and other evidence-based service approaches is preferred. Proficiency with HMIS and computer systems, strong documentation and communication skills, and the ability to manage multiple cases effectively are required. Successful completion of required background checks and pre-employment screenings is also required.
