Provides nursing care in accordance with physician’s plan of treatment, individual plan of care, as authorized by client’s payer source. Observes, records and reports reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician. Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment. Participates in education programs, quality assessment activities, nurse’s meetings and other staff meetings as required. Provides oversight for making patient and personnel assignments. Provides oversight for referrals. Responsible for initiating patient discharge planning upon patient admission to care. Accurately and timely complete Discharge Summary at the time of patient discharge from agency services. Performs HHA supervisory patient visits as required. Performs PRN visits, work rotating weekend shifts, and available on call as required. Participates in quality improvement program to identify and report problems, identify problem indicators, and perform chart audits and review. Works with DON to communicate patient status and problems daily. Travel tp patients homes and outreach locations to provide care under the direction of the DON. Maintains compliance with all Phoenix policies, procedures, and regulations, including all federal and state laws. Performs all other duties as requested by supervisor. JOB SUMMARY The Minimum Data Set (MDS) Coordinator is responsible for the MDS assessment process completion, care planning process, and scheduling of MDS assessments. The MDS Coordinator assures maximum reimbursement by: determining payment coverage of each resident from admission to discharge; and by strategically setting Assessment Reference Dates (ARD’s) for the MDS in collaboration with therapy services within the Omnibus Budget Reconciliation Act (OBRA) requirements. JOB RESPONSIBILITIES Administrative functions May be involved in the pre-admission assessment in collaboration with the DON, Admissions Coordinator and HIM to determine coverage level for Skilled Nursing Facility (SNF) stay. Meets routinely and as indicated with the business office regarding billing requirements. Conveniently located in the North Bay, SRBHH serves adolescents and adults. We treat both mental health and co-occurring psychiatric and substance abuse conditions. POSITION TITLE: House Supervisor PAY RANGE: $66.00-$77.25 Per Hour REPORTS TO: Chief Nursing Officer HOURS: Day Shift 7am-7:30pm DESCRIPTION OF POSITION: Under the supervision of the CNO and/or Nurse Manager, the House Supervisor performs key responsibilities and meets position requirements as outlined in the hospital’s RN job description.  Outside of regular business hours, the House Supervisor is also responsible for overseeing admissions and discharges, unit operations, fiscal and staff management, and overseeing patient care services. The House Supervisor supports and directs Nursing staff assignments and facilitates communication with other disciplines, departments and community resources. The House Supervisor maintains knowledge of, participates in further training provided, and understands the potential risks regarding occupational health hazards (e.g. bloodborne pathogen exposures).   KEY RESPONSIBILITIES: Works closely with Assessment and Referrals (A&R) to provide oversight of admissions and discharges. Conducts real-time audits of all seclusion or restraints interventions on assigned shift. Monitors inventory of office, medical, and patient care supplies. Requests replenishment of supplies as needed. Alerts the Administrator on Call (AOC) when urgent or extreme events occur. Examples include, but are not limited to, AWOL patients, 911 or urgent ambulance transfers, patient injuries, staff injuries, and incidents involving law enforcement. Gathers and compiles data for daily shift and routine reporting. Routine reports include Shift Summary, Bed Control, and Incident Reports.  Prepares and investigates Incident Reports (IR) in an accurate and timely fashion; follows up with staff education to prevent reoccurring incidents.
