IBH Care Coordinator (Integrated Behavioral Health) Location: Oakland, CA Schedule: Monday – Friday, 8:00 a.m. – 5:00 p.m. Salary Range: $66,000 – $80,000, depending on experience Position Summary The IBH Care Coordinator plays an essential role on the Integrated Behavioral Health (IBH) team, supporting care coordination efforts across primary care, behavioral health, housing, and social service systems. This position is ideal for an Licensed Mental Health Counselor Associate seeking hands-on experience in an integrated care environment focused on improving health access and outcomes for low-income communities in Alameda County. Supervision toward clinical licensure is available for qualified candidates. Key Responsibilities Care Coordination & Program Support Deliver in-person and telehealth-based care coordination for individuals with behavioral health and social support needs Conduct comprehensive client assessments and contribute to individualized care planning Facilitate warm handoffs and referrals to behavioral health, medical, housing, and community-based resources Monitor engagement and support follow-up care to maintain service continuity Collaborate with internal care teams and external partners to address client needs holistically Participate in multidisciplinary team meetings, case reviews, and service planning sessions Document services and progress accurately in the electronic health record (EHR) Assist with data collection and service tracking to meet reporting requirements Compliance & Training Ensure compliance with Medi-Cal, HIPAA, and internal documentation protocols Attend required trainings, community forums, and IBH-related meetings Support program evaluation activities by monitoring outcomes and service delivery Contribute to outreach, resource navigation, and sustainability efforts Requirements Registered Associate Clinical Social Worker (ASW/ACSW) in the State of California Bachelor’s or Master’s degree in Social Work, Psychology, Public Health, or a related field Minimum 1 year of relevant experience in behavioral health, case management, or social services Familiarity with Alameda County systems of care, including Medi-Cal and community-based resources Understanding of trauma-informed and culturally responsive service delivery Strong communication, time management, and interpersonal skills Ability to work collaboratively with both clinical and non-clinical team members Preferred: Experience in integrated or primary care settings Proficiency in an electronic health record system (SmartCare or similar) Bilingual skills (Spanish or other languages) highly valued Reliable transportation and valid CA driver’s license Work Environment Hybrid community-based and clinic-based setting Occasional travel within Alameda County Use of standard office and telehealth equipment May require occasional evening or weekend flexibility Supervision & Support Clinical supervision available to support licensure hours Ongoing training and mentorship provided through internal and external partners Opportunity to grow within a supportive, multidisciplinary team environment Equal Opportunity Statement This position complies with all federal, state, and local regulations including HIPAA, CMIA, and Medi-Cal standards. As a Senior Director of Regulatory Affairs, you will manage a team of regulatory professionals who analyze new regulatory requirements, work closely with the business to understand the impact and obligations of those requirements, and support the business in understanding the evolving regulatory landscape. With wide latitude for individual initiative and decision making, the Senior Director of Regulatory Affairs is responsible for providing leadership and oversight of the Regulatory Affairs unit relating to all products offered by Health, as well as leading other projects assigned by Executive Leadership.   Provide leadership and expertise in analysis and interpretation of complex and voluminous enacted laws (state and federal) across the portfolio of businesses. Direct and supervise the Regulatory Affairs unit to ensure that all new regulations and product contract updates are disseminated throughout the organization.   Provide regulatory support to the organization, including providing regulatory interpretation and gaining clarification from regulatory bodies.  Proactively communicate with internal divisions to become and remain educated about key regulatory issues affecting these areas.   Lead continuous improvement of communication processes to better inform the enterprise of new legislation and regulatory agency directives.  Assist the Compliance teams with understanding of Federal, State and Local regulations and product contract requirements. Drive team success, engagement, and development to resolve highly complex or unusual business problems.  Requirements Bachelor's degree required Minimum ten years of experience working with Medicare and Medicaid laws/regulations, including having held a senior position, in a managed care compliance or regulatory department and must possess detailed knowledge of the rules governing managed care health plans. Thorough knowledge in the application of health law and policy, managed regulation and compliance programs and/or the fundamentals of healthcare organization, administration and standards, regulations and law applicable to health plan operations. Knowledge of business and human resources administration principles, management functions, management processes and functions of health plan departments. Ability to direct and supervise personnel.
