Housing Retention Specialist, CERF - Mid-North County (North Santa Barbara County)
The Housing Retention Specialist for the New Beginnings’ Encampment Resolution Project will engage, assess and assist eligible clients as referred by our Encampment Resolution Project, by partnering with clients to build skills and access resources and support systems necessary to maintain their housing. The Housing Retention Specialist will meet with clients to provide supportive services and to promote housing retention, community integration, life skills, and improved health and wellness. The Encampment Resolution Project will work to resolve 21 vehicle encampments county-wide. The Housing Retention Specialists will report to the Encampment Resolution Project Coordinator and the Vehicle Encampment Resolution Program Manager. The goal of the project is to house 150-300 individuals residing in these encampments. We are hiring two Housing Retention Specialists - one based out of our Santa Maria office and one located in our Lompoc office. The Housing Retention Specialists will be in the field at least 75% of the time. Position Duties: • Facilitate the connection and enrollment of clients into eligible and applicable programs and resources; • Support clients, collaboratively working with their landlords, neighbors, and existing case management services as needed in order for the client to thrive within their housing; • Assist clients in relocating to more appropriate housing if needed; • Strive to recognize the best in each client and to support the meaningful change they seek through building relationships and utilizing motivational interviewing techniques and strength based case management techniques; • Provide client engagement by welcoming the client to their new community, addressing basic needs, establishing trust and assisting the client with understanding the services that are available to them; • Meet with clients on a regularly scheduled basis and document growth and strength in progress notes and in Fulcrum and HMIS; • Provide home and field based services as appropriate and transport clients as needed to essential appointments that support their well-being; • Assist client with navigating and abiding by their lease obligations; • Support clients with learning and practicing fiscal responsibilities; • Assist client with their physical and mental health needs by providing support and linkage to appropriate services and/or our internal multi-disciplinary team; and • Participate in client review meetings and provide input to community partners as appropriate. Case Manager – Supportive Services for Veteran Families Program (SSVF), North County, New Beginnings. 40 hours per week/full-time, non-exempt Case Manager position. Program provides eviction prevention and rapid rehousing case management for veterans and their families in North County who are struggling with housing instability. Mental health background/intern registration, post-doc, or licensure preferred but not required. Bi-lingual is helpful but not required. Veteran status or previous experience working with veterans is a huge plus. Responsibilities and tasks include a wide range of case management activities working with the adult veteran homeless population, including: • Case management tasks helping veterans and their families secure housing and look for available housing/rental units; • Coordinating housing with landlords; connecting the veterans with community resources and government and health benefits; developing case plans with clients in order to help them stabilize and resume healthy functioning; • Resume preparation, job coaching and connection to employment opportunities; • Mental health triage; • Connection to resources; • Collection of data and shared responsibility for tracking outcomes and program performance measures; • Community advocacy and participation in collaborative task force activities; • Presenting and providing information to community members and organizations about the program; and • Coordinating with inter-agency programs and activities. In addition, this position will serve as the SSVF representative at North County Veterans Treatment Court and at the Coordinated Entry System Case Conferencing Meetings. This is not a clinical position but some clinical hours may be gained towards licensure and weekly supervision is provided.
