TEMP TO HIRE JOB DESCRIPTION This is a responsible, non-supervisory position requiring familiarity with various veterans/government programs and insurance coverage related to services for veterans. Duties include but not limited to: Implements and coordinates the case management in the region to ensure prescribed activities are carried out in accordance with specified objectives of the Veteran Directed Care Program; Provides initial assessments, technical assistance and develop spending plans for consumer directed services; Conducts and documents ongoing follow-up contacts and home visits. Serves as a liaison between CLIENT, Veterans Affairs (VA), Fiscal Management Agencies and social service agencies. Attends meetings, conferences, workshops and other in-service training programs related to the case management program and benefits counseling; Assists in developing the area plan and any necessary amendment program/benefits counseling program; Liaison between CLIENT, vendor regarding invoices; Completes related monthly reports; Performs other job related duties as assigned. Desired Degree/Job Experience: High School Graduate or GED with some college courses in Behavioral Sciences, Social Services, Public Administration or related field; Two (2) years of full-time Aging, Social Services experience can be substituted for one (1) year of preferred education or any equivalent combination of experience and training which provides the required knowledge, skill, and abilities.. Other Qualifications: Valid Texas driver's license and reliable transportation required; Travel within 13 Counties of CLIENT. Provides outreach to those clients who may benefit from other programs developed within the Area Agency on Aging; Implements the case management program through the identification of clients; Proficiency in basic computer skills, Windows, Microsoft programs; Database utilized for maintenance of electronic files; Bilingual Spanish/English preferred.
