Through person-centered care, housing navigation, and community-based supports, we empower each individual to build a foundation for recovery, self-sufficiency, and a better quality of life. ____________________________________________________________________________________________ Position Summary The Case Manager provides outreach, assessment and engagement to participants of the SOR Rapid Rehousing Program.  Support participants in residential substance use treatment transition out of treatment into independent living. Provide ongoing case management support. Work with participant to assess their needs, find safe and affordable housing, manage their income, and strengthen community support. Complete documentation in timely manner. Perform responsibilities professionally, in accordance with MHA’s Core Values (Respect, Integrity, and Compassion), policies, practices, program funding and applicable regulatory agency guidelines.  Pay Rate: $20 an hour Schedule: Monday through Friday 9am-5pm (40h), with some flexibility required Key Responsibilities Assess participant eligibility and gather required documentation for entry into the program. Engage participants using a trauma-informed, person-centered approach to identify goals and support community integration. Assist with housing searches, applications, landlord meetings, and apartment viewings. Support participants transitioning from treatment to independent living, including ongoing tenancy and family support. Provide tenancy education and assistance, including landlord negotiation and compliance with lease responsibilities. Help participants develop safety and relapse prevention plans, and connect to appropriate recovery supports. Assist with employment, education, money management, and accessing benefits and community resources. Coordinate medical, dental, psychiatric appointments, and connect participants to medication management when needed. Maintain complete and timely documentation, including incident reports, service notes, and use of agency systems.
