What You Will Do As a Nurse Care Coordinator (RNCC), you will assist with the coordination of care for pediatric and/or adult patients of IHI. This includes new patient coordination of care, communication, and education with patients, families, and healthcare providers. This also includes supporting standard workflows, patient tracking, national surveillance, data capture, and disease specific interventions to optimize patient outcomes. The Opportunity Facilitate prompt communication of nursing assessments and recommendations to providers, team members, patients and families to ensure all patient needs are met, i.e. medical and surgical care planning Coordinate management and data tracking disease specific care standards utilizing and contributing to national surveillance and outcomes Analyze trends, gaps, and needs in patient care to recommend and implement programs to enhance improve patient services. Identify at-risk family members of affected patients and coordinate screening/testing Provide and develop culturally competent education for patients, families, and staff applying health literacy guidelines Represent organization at patient/provider conferences and events Requirements Completion of an approved nursing program Minimum 2 years of experience working with patients with a chronic disease Valid Indiana Driver’s license and automobile insurance This position is open to residents of the State of Indiana, and requires routine presence at our Center in Indianapolis, IN. Frequency of on-site presence is dependent upon role and department needs. Benefits Why join our team?
