CFTSS (Children and Family Treatment and Support Services) program provides government-funded support for children with mental, emotional, and social health challenges. DSS staff will work on specific goals set by clinicians to improve children’s mental, social, and emotional health. Those with experience with in foster care, addiction, mental health, or related human services backgrounds with children encouraged to apply. This role is ideal for a new graduate looking to gain experience in geriatric care while working in a flexible and supportive environment. Key Responsibilities - Conduct telehealth assessments and consultations for senior patients. - Perform in-home visits to assess, diagnose, and treat patients in their living environments. - Manage acute and chronic conditions with a focus on geriatric primary care. - Develop and implement personalized treatment plans to improve patient health outcomes. - Provide patient and caregiver education on chronic disease management, medication adherence, and preventative care. - Coordinate with physicians, specialists, and other healthcare providers to ensure comprehensive care. - Document all patient interactions in an electronic health record (EHR) system. - Recommend and facilitate preventative screenings and wellness programs. - Participate in team meetings and case discussions to optimize patient care strategies.
