Immediate opening for experienced home health RN/LVN to work in house in our North Hollywood Office with occassional trips to patients in the home(rare). Mostly office experience to do phone triage, assist in completing 485s/Plan of Treatment (computerized program Soneto which codes and types treatment plans), review nursing and multidisciplinary notes, review Evals and Recerts for non Medicare Home Health Agency (no oasis). Mainly private duty patients. Position is full time with health benefits after 3 mo. training period & 401K after one year. HOURS ARE MONDAY THROUGH FRIDAY FULL TIME 10-7pm OCCASIONALLY WE MAY NEED YOU TO WORK 7am - 4pm AS NEEDED. “OFFICE POSITION ONLY” RATE IS $30-45/hour LVN $60-75/hour RN Position Responsibilities: Initial Intake/Insurance Verification/Give Field Nurses Clinical Information Review Field Staff Clinical Progress Notes Give orientations and inservices to all field staff Telephone triage - new intake, order medical supplies, coordinate care of patients Infection Control Log and write incident reports Quality Assurance/Quality Improvement activities including preforming quality management activities on charts Participation in QA/QI meetings After Hours Phone Triage (share with existing management staff) OFFICE WORK - WORKING TOWARDS ACHC CERTIFICATION YOU WILL WORK WITH existing Supervisory RNs. Positive upbeat individual with patience and who is detail oriented who can speak with respect to management staff and field staff. Prefer "people person" who can spend time with field staff and encourage them both in person and over the phone - "praise them to success". Patient person who can work with both the skilled private duty staff and the non skilled including companions, CNAs, CHHAs. Must work with staffing coordinator and clerical staff to supervise basic activities. We need good computer skills. PREFER PREVIOUS HOME HEALTH EXPERIENCE OR SIMILAR RN SUPERVISOR & QA/QI EXPERIENCE.
