Concern Housing is seeking a Program Supervisor to be responsible for the overall supervision and management of the program site to help promote the supportive tenant’s growth, safety, and the effective delivery of support services. Position: Program Supervisor Location: Bronx, NY (Norwood Terrace) Schedule: Tuesday - Saturday; 8am - 4pm Salary: $68,000/yr. Responsibilities Include but are not limited to: - Assist with the supervision and coordination of all direct care services, including staff scheduling, supervision and crisis intervention where needed (including 24/7 cell phone coverage) - Oversee all intake procedures, including screening of applicants and monthly outcome reporting to HRA and DHS. Maintain program occupancy rates (>95%) - Compliance with LIHTC regulations, including eligibility determination, annual certifications and any other reporting requirements, as necessary - Monitoring the supportive tenants’ charts for timely submission of paperwork, and qualitative review of progress notes, services plans and reviews, functional assessments and other required documentation - Cooperation with the Property Management department re: site maintenance and safety issues, as well as the accounting department to ensure monitoring of rental payment/ arrears and assisting special needs tenants with securing and maintaining entitlements - Provide staff training and development to ensure suitable implementation and evaluation of evidence-based best practices. Evaluate staff performance and promote professional growth - Conduct continuous needs assessment for program development and evaluation and quality assurance. Closely monitor health care outcomes of special needs tenants - Collaboration with local hospitals and healthcare/care coordination providers through committees, special projects, etc., as well as community outreach - Coordination of Resident Advisory Council meetings and liaison with the Council - Other tasks as assigned by the Senior Program Director - Remain current with all Agency required compliance documents and trainings Requirements: Master’s Degree in Psychology, Social Work or related mental health field with at least 3 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good leadership and supervisory skills, good verbal and written communication skills and the ability to make sound judgments regarding client care. You will receive support from Enriching Lives to assist you in providing services in your own home such as initial and continuing education, the support of a multidisciplinary team, 24 hour on call professional support, and overnight relief support. Enriching Lives staff will visit you and the Individual regularly to provide support and ensure that the Individual’s goals are being met. At a minimum, you must have 1-2 spare bedrooms, a safe and pleasant living environment, reliable transportation, a high school diploma or GED and a designated backup Family Home Provider. All adult members of the household and frequent visitors to the home must have a clear criminal history and a DMV record without any major violations. Enriching Lives is highly committed to making successful matches and placements in the home of people who become Family Home Providers with our agency. While there are never guarantees for placements, our success rate is extremely high.
