Description

Coordinate substance use, mental health, medical and access to medications to treat opioid use disorders (MOUD). Complete intake process for all new clients. Ensure ongoing discharge planning and referrals which support ongoing access to MOUD and recovery support services. Monitor and ensure patient compliance with treatment goals and reduce barriers to care. Document all patient interactions/services in the appropriate electronic health record. Monitor the collection of urine specimens and toxicology results. Establish and maintain community partnerships which ensure continuity of care for patients and increases community access to MOUD. Makes recommendations on matters concerning the physical plant operations, workflow, and resources. Treats patients in a manner that preserves and protects patient autonomy, dignity, confidentiality, and patient rights. Performs any additional duties, tasks and responsibilities that may be assigned, as needed. Requirements High School Diploma or equivalent required; Bachelor’s Degree preferred. Two (2) years of case-management or counseling experience working with substance use disorder and/or homeless population required. CASAC, CASAC-T, CARC, or CRPA required Proficiency with computer operation (Microsoft Office, Electronic Health Records, and Outlook programs).? Excellent written and oral communication skills. Ability to interact with service recipients of diverse social, cultural, and economic groups. Ability to remain calm, focused, and helpful under stress. Bilingual English/Spanish preferred. JOB SETTING: · Functions within a thirty (30) foot motorized Mobile Medication Unit. · Must possess sight, hearing and digital senses or prosthetics that will ensure the completion of the essential functions of the Client Care Coordinator, Mobile Medication Unit. · Temperature controlled mobile medication unit for inclement weather. · Must lift maximum 20 lbs.

We are looking for candidates who want to travel throughout the county to connect and help our unhoused community members. The outreach specialist will be expected to engage individual’s experiencing homelessness and assist them in accessing self-identified needs, such as, health, housing, behavioral health and basic needs. Provide transportation resources to access treatment services and/or support in getting to an appointments. 7. Provide on-going education and advocacy for individuals experiencing opioid use disorder, including information on overdosing, Narcan, and Fentanyl testing strips. 8. CASE MANAGEMENT Interested parties should understand diverse populations, including those involved with multiple barriers to reentry, employment, and self-sufficiency. This is a challenging position for someone with experience with data collection, group facilitation, peer-to-peer mentoring, one-on-one case management, and a strong leader with good social skills.
