You are what make us amazing!  Responsibilities:  Provide direct pediatric therapy services in a home health setting  Manage and grow a personal patient caseload  Collaborate with the clinical director and recruiting team to expand the local therapy team  Provide support, training, and mentorship to newly hired therapists  Ensure compliance with all company, state, and federal policies and procedures  Participate in case conferences, team meetings, and clinical development  Transition into a team management/administrative role as the caseload and team grow  Requirements Current licensure in the state of Texas as a PT  Minimum of 2 years of pediatric experience, preferably in home health  Demonstrated leadership skills or prior experience in a lead or supervisory role  Strong communication, organizational, and interpersonal skills  Ability to manage time effectively and work independently  Passion for working with children and their families    Preferred Qualifications:  Previous experience building or managing a therapy caseload/team  Bilingual (English/Spanish) a plus  Knowledge of EMR systems and therapy documentation standards  Benefits Weekly pay - We pay our team weekly so you can enjoy a steady income. Also, I am looking for someone to work alternate weekends with my other weekend caregiver. The weekend shifts are 3 hours in the morning, 1 hour for dinner, and 1.5 hours for bedtime. Showers Are completed with the help of a transfer system which limits lifting. I have worked with caregivers successfully from ages 18-70+. As long as you are physically able to use my assistive devices we can get all my caregiving activities completed successfully. We are seeking an experienced Qualified Intellectual Disability Professional (QIDP) to support our Vista Care operations in Lincoln, IL. JOB SUMMARY The Qualified Intellectual Disability Professional (QIDP) is responsible for implementing personal goals and outcomes for individuals supported at Vista Care. In partnership with the staff and individuals supported, the QIDP works to ensure that there is progress being made towards these goals.  JOB TYPE: Full-time SCHEDULE: 8/10 hour shift LOCATION: Multiple Locations RESPONSIBILITIES: Implement individual program plans in accordance with applicable State and Federal regulations, and coordinates residential direct care services as defined by State and Federal regulations. Case Manager will be assigned a caseload of developmentally disabled adults. Ensure that safety, health, and happiness needs of each client is met. Conduct staff training as necessary to ensure care is optimized. Interact regularly with client's guardians. Coordinate efforts of the Interdisciplinary Team and program staff to ensure that each Individual is receiving optimum services, which meet his/ her individual needs. Observe individuals, review data and progress, and revise programs based on individual needs and performance. Take action as necessary to ensure each individual's care needs are met. Develop, implement, review, and document each resident’s individual program plans. Conduct continual training for caregivers to ensure program plans / behavioral plans are adequately executed and resident care is optimal. Assess and resolve emergency behavioral and medical needs of residents, which may necessitate night, evening, and weekend hours. Complete all required documentation in a timely manner, including monthly and biannual reports on each individual within caseload. Schedule, coordinate, and transport individuals to medical appointments. Make arrangements with day training agencies and or caregivers as need to ensure Individuals are ready for appointments. Complete all required documentation and Medical appointment follow-up activities. This includes scheduling all required follow-up appointments and testings. Fully document within individual files and Therap (including scanning all documents) each medical appointment and all follow up activities. Ensure medical files for all individuals are current at all times. Requirements Must have at least one year of experience working directly with people with developmental disabilities, meet the minimum federal educational requirements for a QIDP outlined in 42 CFR 483.430, and must provide documentation of both education and experience. Requires a minimum of a bachelor's degree in social work, psychology or related field. Requires a minimum of 1 year experience working with developmentally disabled adults. Vista Care can provide all training required to obtain QIDP certification. Experience in Person-Centered Supports preferred. Required to take 12 hours of continuing education maintain QIDP status.​ Familiarity with CILA rules preferred.
