Amazing Care Pediatric Home Health is seeking a dedicated and compassionate Pediatric RN Case Manager to join our team in the Dallas/Richardson area. As a vital member of our healthcare team, you will be responsible for providing comprehensive case management services to pediatric patients and their families. This role involves coordinating care, advocating for patients, collaborating with healthcare providers, and ensuring that patients receive the highest quality of care. Requirements Conduct initial assessments of pediatric patients to determine their healthcare needs and develop individualized care plans. Coordinate and monitor patient care to ensure timely and appropriate services are provided.
