POSITION TITLE:         Program Associate - CalFresh Promotor                              DEPARTMENT:               Community Initiatives REPORTS TO:              Director of Nutrition and Food Security Programs LOCATION:                  Los Angeles, Downtown CLASSIFICATION:        Non-Exempt, Full-Time HOURLY RATE:           $20.00 PERTINENT INFORMATION: This position will service West LA, Metro, South Los Angeles Work Shift: Monday - Friday, 9:00AM-5:30PM This position to be filled as soon as possible Responsibilities and Initiatives Implement day-to-day program tasks, including enrolling individuals into food assistance programs (such as CalFresh and/or P-EBT), conducting community workshops on local food resources, communicating and maintaining partnerships with community members & partners, and other duties as assigned. Monitor and case-manage individuals enrolled in food assistance programs. Stay up to date with LA City and County health and safety-net programs like CalFresh, SNAP-Ed, Medi-Cal and others, and how they can support community members and NHF programs. Represent NHF at community meetings and build relationships with community members to increase NHF’s program participation. Coordinate and/or assist with community events that promote programs, services, and NHF overall. Maintain records of program deliverables, such as attendance sheets and event reach, and input data in evaluation dashboards. Provide support to other team members, including but not limited to preparing workshop & class materials, reviewing program lesson plans & curricula, assist with program planning, etc. Always follow NHF policies and procedures. Requirements At least 3 years of experience conducting community outreach and education as a certified or trained Community Health Worker or Promotora. Knowledge of community resources, including public benefits, nonprofit organizations, and local businesses. Experience working effectively with diverse communities. Comfortable with public speaking and leading group activities Strong team player able to work effectively and efficiently with others toward common goals. Fluent in English and Spanish, or other commonly used language in Los Angeles County. Reliable transportation! This position will be required to visit multiple different sites over the course of a week and will need transportation. Position Title: Direct Care Counselor FLSA Status: Non-exempt Department: Ke Kama Pono Supervisor: Program Director Primary Purpose: Provide direct care and supervision to youth according to the foundation’s policies and procedures, licensing and accreditation requirements. Essential Duties: • Supervise activities for residents that will increase their cultural awareness and enhance their self-esteem; • Work with the Lead Direct Care Counselor to oversee residents in their day to day routines in the house; • Assist in the delivery of services, and activities for the residents in the program; • Review and summarize miscellaneous reports and documents; • Prepare documents and reports as necessary. Other Duties: Other duties as assigned by Program Director or direct supervisor. Work Hours: • Sunday through Saturday, work hours vary depending on program needs or otherwise determined by the supervisor; • Position requires regular and predictable attendance; • Some events on evenings or weekends at various times. Working Conditions: • Office setting; • Travel in program and/or personal vehicle to and from program sites. Equipment Use: Program vehicle, computer and/or lap top, printer, scanner, copier, fax machine, office and/or work cell phone/Bluetooth, paper cutter, nametag maker, voice recorder, speaker system, video and camera equipment, microwave, ice machine, and stove. Minimum Qualification Requirements: • High school diploma, or equivalent, and experience working with youth required; • AA or BA degree in social work or related field preferred; • Must have a valid driver's license, a safe and responsible driving record; • Must participate in Child Abuse and Neglect, State criminal and FBI criminal clearances, and subsequently meet DHS standards regarding these clearances; • Must have current CPR/First Aid certification that will be updated annually; • Must have current Crisis Prevention Intervention (CPI) certification that will be updated annually; • Must have current TB test completed that will be updated annually; • Must pass a physical examination requirement.

The Street Heath Outreach Program is an innovative project which identifies homelessness, chronic health and mental health needs in the Central Alameda County. The program will assist people in attaining housing stability and improve health and well-being. Using a multi-disciplined team approach, the program provides street and venue-based outreach and engagement, assessment, health services and triage, case management, as well as linkages to housing agencies and mental health services. The program will work with all participants to develop and carry out action plans that improve health and wellness. Under the supervision of the Director of Population Health, the Street Health Outreach Community Health Worker will work directly with participants to access housing programs, assist with enrollment and/or maintenance of Medi-Cal or HealthPAC, and provide navigational support to successfully engage participants to a medical home. Other activities include health education, care plan development, public benefit linkage and assisting the participant with accessing TVHC health care services. Creates a care plan that reflects the client’s need based on outreach assessment. Coordinates linkages to various community resources to meet these needs and works from an interdisciplinary approach. Provides case management services in a culturally, linguistically and age appropriate manner. Serves as a liaison between the client, community and health and social services. Assists clients with enrollment and maintenance of MediCal and Medicare eligibility as well as with other benefits such as CalFresh, GA and SSI. Provides culturally competent, trauma informed health services. Maintains client information and case documentation in a confidential and professional manner. Completes Coordinated Enrry Assessments for individuals seeking housing. Maintains client data and responsible for updating county database on a monthly basis. Adheres to organizational policy and procedures. Builds and maintains appropriate relationships with staff, encampment leaders and Housing Resource Centers. Supports a positive work environment and fosters teamwork. Represents Street Outreach team at county and community based organization events and meetings. Participates with other staff in multidisciplinary meetings and other staff processes to achieve program development, staff development, program evaluation, program operations review, and other activities as appropriate. Requirements High school diploma, or equivalent required. Associate of Arts Degree preferred. Valid Cardio Pulmonary Resuscitation Card (CPR) within three months of hire. Minimum of 1 year experience providing outreach to chronically unhoused individuals preferred. Experience with Electronic Medical Records (EHR) preferred. Lived experience of homelessness valued. Qualifications: Bilingual; English/Spanish preferred. Be highly dependable and independent. Good communication; ability to work well within a team and independently. Proficiency in computer operations, navigational skills and comfortable with Internet-based applications. Sound decision-making skills including problem solving. Professional demeanor.
