Are you seeking a Case Management position? Come work for Springwell and help older adults age-in-place and adults with disabilities receive care in the community. DESCRIPTION: Work collaboratively as part of an interdisciplinary team that provides in-home services to older adults and individuals with disabilities. Make home visits, conduct comprehensive assessments, and participate in the development of person-centered care plans. QUALIFICATIONS: Bachelor's Degree or higher. Applicants without a Bachelor's Degree may be considered if: The applicant has an Associate's Degree and at least 3 years' experience working with elders or providing case management services; OR The applicant has completed at least 50% of coursework required for a Bachelor's Degree and agrees to complete the remaining coursework within a 3-year period. A strong desire to work with seniors and individuals with disabilities. Excellent organizational skills and the ability to manage constant change with a positive attitude. Experience working with computers, entering data, and running reports. Applicant must have a reliable car, current driver's license, and safe driving record. My name is Eric. I am a 47-year-old man with cerebral palsy. I am looking for someone who is able to work weekday bedtime from 9:00 PM until 10:30 PM. Also, I am looking for someone to work alternate weekends with my other weekend caregiver. The weekend shifts are 3 hours in the morning, 1 hour for dinner, and 1.5 hours for bedtime. I do not take advantage of my caregivers. I treat all caregivers with respect. I am looking for someone who is motivated, kind, and available to help with mornings, dinners, and bedtime. Responsibilities include: transfers with a lift, toileting, hygiene (showers, shaving, teeth brushing etc.) cooking meals, feeding me, and helping me get ready for my day in the morning and bedtime. Showers Are completed with the help of a transfer system which limits lifting. I have worked with caregivers successfully from ages 18-70+. As long as you are physically able to use my assistive devices we can get all my caregiving activities completed successfully. I am willing to work with anyone who is responsible, dependable, and trustworthy. I need someone who take responsibility seriously and can communicate clearly and honestly. My morning shifts are 3 hours long. They usually start between 7 AM and 8 AM. We can discuss the schedule that will work best for you and me both. My dinner shifts are one hour long. The timing of dinner is flexible. I don't expect anything fancy. I am easy to please. My bedtime shifts are either one or 1.5 hours depending on whether I do a shower during the shift or not.
