Description

Coordinate substance use, mental health, medical and access to medications to treat opioid use disorders (MOUD). Complete intake process for all new clients. Ensure ongoing discharge planning and referrals which support ongoing access to MOUD and recovery support services. Monitor and ensure patient compliance with treatment goals and reduce barriers to care. Document all patient interactions/services in the appropriate electronic health record. Monitor the collection of urine specimens and toxicology results. Establish and maintain community partnerships which ensure continuity of care for patients and increases community access to MOUD. Makes recommendations on matters concerning the physical plant operations, workflow, and resources. Treats patients in a manner that preserves and protects patient autonomy, dignity, confidentiality, and patient rights. Performs any additional duties, tasks and responsibilities that may be assigned, as needed. Requirements High School Diploma or equivalent required; Bachelor’s Degree preferred. Two (2) years of case-management or counseling experience working with substance use disorder and/or homeless population required. CASAC, CASAC-T, CARC, or CRPA required Proficiency with computer operation (Microsoft Office, Electronic Health Records, and Outlook programs).? Excellent written and oral communication skills. Ability to interact with service recipients of diverse social, cultural, and economic groups. Ability to remain calm, focused, and helpful under stress. Bilingual English/Spanish preferred.
