What You'll Do: Execute incoming calls, assess client/patient needs, and identify appropriate comprehensive and individualized services to serve each client/patient best. Collect appropriate documentation to support and determine eligibility for program services. Assist client/patient with scheduling dates for appointments or classes. Process patient referrals for a variety of services, including medical and dental, as well as programs, including behavioral health. Monitor and document active referrals and client participation. Provide program administrative support such as data entry, client file management, report development, and contract compliance. Coordinate and promote YVFWC programs.
