Keeping Families Together is a long-term program aimed to assist DCPP involved families challenged with homelessness/housing instability putting children at risk for out-of-home placement or delaying reunification. KFT utilizes a housing-first model to work with our families in obtaining stable, subsidized housing. The program focuses on the following to help families overcome the obstacles that prevent the family unity they desire: 1) Reunification; 2) Housing Stability; 3) Vocational Counseling; 4) Substance Use Services; 5) Socialization and Self-Sufficiency; 6) Parenting Skills; 7) Strengthening Families Curriculum. Position Title: Clinician Position Type: Full Time; 40 hours weekly Benefits Eligible: Yes Location: Toms River, NJ Department: Keeping Families Together (KFT) Salary: $45,000 - $50,000 Responsibilities: As a member of a multi-disciplinary treatment team provide direct care as delineated in the CTP Provide therapy as clinically indicated for individuals and family units Engage all consumers and their family members in treatment planning and all areas of service delivery Assist consumers with ADL's through modeling, direct assistance and coaching Provide assessment of client's emotional well-being, clinical symptomology, behavioral changes and medication compliance. About the Role The Youth Life Coach/Case Manager position is responsible for the provision of Life Coach/Case Management and follow-up services and will prioritize serving female youth aged 14 – 18.
