Role: We are seeking compassionate and dependable individuals for our Direct Support Professional (DSP) positions. You will provide care for our residents/participants (adults with IDD) at our homes and Adult Day Health (ADH) centers under the guidance of the home and center managers. The Aging & Disability Services Care Coordinator III’s primary responsibility is to support older adults, individuals with disabilities, and their caregivers to achieve their goals for community and independent living. Care Coordinators will conduct initial assessments with potential clients to confirm program eligibility and identify the client’s goals.
