Social Worker - Early Start San Mateo Office Starting Salary Bachelor's Degree: $58,083 - $69,700 Master's Degree: $64,008 - $76,810 Are you looking to make an impact and do work you are passionate about? GGRC is currently hiring for Social Workers in out Early Start Division. This is a Hybrid flex schedule role offering the opportunity to work in the community, from our office located in San Mateo County and from the comfort of your own home. What you’ll Do: Early Start does the needs assessment and works with the families of clients aged 0 - 3 to develop, implement, and coordinate the client’s Family Service Plan, acting as an advocate and service coordinator to the clients and their families. Local travel within Counties of San Francisco, San Mateo and Marin may be required. Occasional statewide travel and access to reliable transportation and many different locations throughout the day in order to perform the outline responsibilities of the position is required. Meet with clients/families and conduct a needs assessment. Develop action plans for community resources and referrals. Provide information to families and community agencies regarding GGRC and services available. Partner with families to problem solve and build on family strengths and natural supports. Monitor child’s developmental status and provide developmental activities to enhance development. Provide intake services and crises management as needed. Act as an advocate with client/family to assure receipt of entitled services and achievement of client’s rights. Educate clients/families on available resources and support and on specific policies/programs such as Medicaid Waiver and Institutional Deeming. Review and ensure that clients receive appropriate services from existing community resources. Evaluate availability and effectiveness of services. Research available community resources and services. Initiate referrals and monitor progress of referrals and of vender services. Participate in community meetings and activities. Participate in staff and other internal meetings and task forces as assigned. Help improve and develop community service by providing appropriate support. Maintain client records, case and Title XIX Progress notes as required. Determine eligibility for further GGRC services. Coordinate transition from Early Start services to local school districts or other community programs. Prepare Special Incident Report and other reports and forms. Ensure compliance with program’s policies and procedures as set forth by the State of California. Ensure that services are provided per relevant regulations. Who You Are You have: Experience in multi-cultural settings and/or multi-lingual capacity; Knowledge of identifying and navigating generic services; Knowledge of principles, objective and techniques of counseling individuals; Knowledge of typical and atypical child development behavior, human growth and development of family relationships; Knowledge of state and federal legislation concerning Early Start regulations and services for High Risk Infants and their families. Good interview skills and team spirit; Ability to balance conflicting priorities and focus on key job outcomes; Ability to proactively stay current with new information in area of expertise; Excellent communication skills in producing documents in a business manner and conveying complex information in a clear, concise and professional manner; Familiarity with MS Office or similar software applications; Creativity and problem solving skills; Organizational skills and time management; Crisis management and decision making; Empathic and able to respect and support cultural awareness. Requirements Bachelor’s Degree from an accredited school of social welfare or related human services field. Master’s Degree preferred; Case Management experience with infants or children under 4 strongly preferred; Fluency in Spanish is preferred for this position. Once housing is secured, the Case Manager will work to link families with services and resources to help them stabilize in their new housing and neighborhood. This follow up case management often includes assisting families with budgeting, learning how to be a model tenant, increasing income, and linkage to other service providers in the community. The Case Manager will serve as the primary point of contact for all assigned client services and provide guidance and support to clients working toward self-sufficiency. This work is fast paced, and the client relationships are often time limited. The Case Manager must be comfortable forming relationships with landlords and other community partners to ensure clients are able to secure housing as their top priority.   Responsibilities: Develop Housing First focused service plans, individualized to the client’s unique housing needs and guide families through the housing location process Proactively and creatively seek new housing opportunities and resources in Fairfax and other localities to assist families with locating housing Empathetically but firmly address any safety issues that may arise during the family’s stay in shelter and work to help refocus family’s efforts on their housing plan. Major Tasks, Duties and Responsibilities: • Work with an individual in their home and in the community holding the interest of your participant at the center. • Implement daily, weekly, and monthly schedules, customized to engage individuals and support them in achieving their person-centered plan and ISP goals. • Accurately assess a situation and respond accordingly. • Redirect inappropriate behaviors and seek positive resolution to individual’s conflicts and difficulties by employing best practices in positive behavioral support interventions and relationship-based approaches. • Utilize non-violent physical crisis intervention tactics if a client presents a danger to themselves or others • Be a positive role model, demonstrating professional attitude, behavior and boundaries in all contexts and environments. • Create and maintain an environment which fosters growth and learning. • Advocate for the person in situations where they are unable to ensure their needs are met or to advocate for themselves (Ex: Laundry, grocery shopping, eating healthy, daily grooming and self-care) • Exercise discretion and professional judgment at all times in keeping with the responsibility carried personally and by the agency for the care and welfare of adults who are developmentally and intellectual disabilities. • Communicate professionally and proactively with clients, parents, family members, residential providers, social workers, employers and peers. • Support the Person-Centered Planning process and ISP development. • Document client progress and capture accurate case notes information in a concise, professional, and accurate manner • Complete paperwork and record keeping of data; case notes, client time sheets, behavioral tracking sheets, social recreation activities, and any other paperwork deemed necessary • Communicate using the Staff Communication Log ensuring a professional tone • Demonstrate effective conflict resolution with other colleagues • Engage individual while encouraging positive peer interactions and healthy socialization • Prompt for tasks of daily living and other skills instruction • Keep Patient confidentiality • Communicate appropriate information to staff relieving your shifts, and to management, to help improve care of client. Minimum Qualifications: • Supported Living Services previous experience highly preferrable or having a caregiving experience preferrable • High School diploma or General Education Diploma (GED), Associates or some College classes • Experience working with adults with developmental disabilities preferred • Have a valid driver’s license and a clean and insurable driving record • Must have a car in the working condition • Have First-Aid Basics and CPR training certification or ability to receive one upon hire • Will undergo criminal background check • Ability to remain professionally detached when engaging emotionally escalated participant(s) by remaining calm and not perceiving client statements or behavior as “personal” attacks. • Ability to demonstrate adequate judgment to make sound decisions and minimize participant risk while in emotional crisis • Must demonstrate creative problem-solving abilities sufficient to navigate unique situations, dynamics, and logistics Working Conditions: • Must have Valid driver’s license and working, safe, car to drive clients where needed • Able to implement non-violent physical crisis intervention tactics if a client presents a danger to self or others • Able to visually observe participant behavior • Able to understand and interpret staff and participant behavior • Able to read, understand and interpret written instructions • Able to communicate and write legibly • Able to learn and administer medication properly through MARS • Able to get clients out for exercise, appointments, and activities • Able to communicate with the weekend manager with updates and daily assessments Opportunity For Independence, Inc.
