• We have two congregate facilities that are 1.2 miles apart, with a maximum of six beds per house. • Provide overall direction and supervision of nursing and caregiving staff, including RNs, LVNs, CNAs, and direct support staff. • Currently licensed by the California State Board of Registered Nursing - BRN • Ensure comprehensive assessments and individualized care plans are completed, implemented, and regularly updated for all residents. • Monitor residents’ health status, coordinate medical care, and respond to changes in condition promptly. • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. • Ensure infection control and disease prevention protocols are implemented and monitored. • Participate in interdisciplinary care conferences and support family communication regarding resident care. • Ensure proper reporting and investigation of incidents, accidents, and grievances. • Conduct routine audits of medical records, medication administration, and resident care plans. • Prepare for and participate in state inspections, surveys, and audits. • Ability to manage multiple priorities in a fast-paced environment. • Must be able to communicate and work professionally and cooperatively with others. • Successfully pass all pre and post-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States • Must visit the facilities a minimum of three times per week and submit complete weekly notes without exception • A detailed list of duties will be discussed during the interview
