About the GRIT Program The GRIT program by the Mental Health Association (MHA) offers residential rehabilitation for individuals with substance use challenges and co-occurring mental health conditions. GRIT provides a structured, supportive environment focused on recovery, personal growth, and long-term stability. Residents engage in individualized counseling, recovery-focused programming, and community activities like education, job training, or employment. With locations in Springfield (men’s programs) and Holyoke (LGBTQIA+ program), GRIT combines the comforts of home with access to vital resources. A skilled, multidisciplinary team works closely with each resident to build a strong foundation for recovery and create personalized aftercare plans for a successful return to the community. _______________________________________________________________________________________________ Position Summary Are you passionate about helping individuals experiencing homelessness secure permanent housing and supportive services?  MHA is seeking a Case Manager to join our dedicated Housing Services team.  In this role, you’ll provide outreach, assessment, service coordination, and housing placement support to individuals transitioning out of homelessness. Pay Rate: $20 an hour Schedule: Monday through Friday 1pm-5pm (20h) Key Responsibilities Engage participants through outreach, enrollment, and assessment Develop person-centered, trauma-informed action plans focused on housing stability Connect individuals to vital services such as mental health care, substance use treatment, legal support, and job training Provide crisis intervention and develop pre-crisis plans Collaborate with community partners, state and local agencies Offer transportation assistance and support participants in achieving their goals Maintain accurate documentation and meet regulatory requirements     Equal Opportunity Statement The Mental Health Association is an equal opportunity employer. Requirements 6+ months of experience working with individuals experiencing homelessness, addiction, or psychiatric symptoms Valid driver’s license, reliable vehicle, and auto insurance Ability to work independently and as part of a team Strong interpersonal, communication, and organizational skills Bilingual (Spanish) and/or Bi-Cultural candidates encouraged to apply
