The Role The home health therapist will play a vital role in delivering exceptional and coordinated care to patients right in their own homes and communities. We are looking for a candidate who not only has a solid background in home health care but also understands state and federal regulations, all while being genuinely passionate about helping patients live their best lives. What You Will Do Conduct comprehensive physical therapy evaluations and create personalized treatment plans to help patients achieve their goals. Provide direct physical therapy treatments and interventions to improve patient mobility, balance, strength, and functional ability. Teach patients and their families how to properly perform exercises and activities to promote recovery and improve mobility. Document patient progress, including changes in condition, treatment plan, and goals, and communicate updates to other healthcare professionals. Coordinate care with other medical professionals to ensure the most eƯective treatment is being delivered. Requirements Graduate from an accredited physical therapy program with a valid Physical Therapy license in the state of Florida. Minimum of one year of experience in physical therapy, preferably in a home health care setting. Demonstrated ability to create eƯective physical therapy treatment plans that promote patient recovery.

Position Purpose Under the supervision of a licensed nurse, the Certified Nurse Aide (CNA) or Patient Care Technician (PCT) provides supportive care to meet the needs of individual patients and serves as a patient advocate within the health care system. Certified Nurse Aide (CNA): Performs the daily comfort and care needs of patients, and assists nurses and other health care team members with patient care procedures and treatments.
