In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare members and other at risk populations. Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care. Perform task to assist in a family home environment for elderly male quad of 71 year old. Family home is mostly taken care from other members in the household. Services like basic cleaning in the home is maintained by family and a few things will be maintain by extra help. Other responsibilities will be taken care of will be by other members in the home. During these time frames the elderly male must have assistance and companionship. Searching for a private caregiver for two young ladies who are intellectually disabled and need 24/7 care. We have a team of caregivers who come into their home and help them live the best life possible. Would like someone that is energetic and wants to be part of their team.
