Concern Housing is seeking an ADL Counselor to be responsible for providing life skills training services directly to clients and is considered a resource in that they should be familiar with the skills and tasks necessary to help consumers learn self reliance skills. The primary task is to provide hands on assistance to clients to help them to develop the skills necessary to maintain independence in the supported housing program. Position: ADL Counselor, Supported Housing Schedule: Monday - Friday; 9am - 5pm Location: Medford, NY Salary: $45,000/yr. Duties and Responsibilities: - Adherence to all policies as set forth in Concern’s personnel manual. - Provide education and skill building assistance across multiple activities of daily living including but not limited to maintaining personal hygiene, maintenance of the living environment, laundering of clothing, travel training, money management, food preparation, nutrition, shopping, basic self preservation skills, communication skills, community integration, and wellness self- management. - Provide individual counseling and support services as it pertains to life skills development. - Work as an integral part of the case management team. Maintain open communication with the case manager regarding skill assessment and goal planning activities. - Develop training tools and techniques in coordination with case manager to address identified client needs. - Maintain all documentation pertaining to visits and activities as consistent with agency policy. - Conduct site visits as needed and directed by Supervisor. - Attendance at weekly supervision and staff meetings. - Remain current with all Agency required compliance documents and trainings. - Other duties as assigned. Requirements: Three years experience working in a similar setting/and or working with people with mental illness. Bachelor’s degree in Psychology, Rehabilitation, Social Work or other related mental health discipline preferred. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care. I am a 36-year-old man with a heart of gold. Looking for an attendant (Male preferred). I have been living in my own apartment in Alameda for the past 10 years. I require a low speaking voice while engaging and communicating with me due to my condition. I require someone who is pet friendly, because I have a cat who I adore. Most of my time is spent in my house on the computer or listening to music and reading. I also enjoy walking in my neighborhood. I need assistance with medication management, meal prep and light housekeeping is required Serious applicants need an insured car to run nearby errands with and without me, mileage reimbursement at the federal rate is provided. Shift is Monday-Friday 7am-3pm. 17 hours of pay will come from IHSS/remainder $19 from Bay Area Support Services, Inc.

We have immediate openings and are accepting applications for Resident Care Assistants for our 3 p.m. to 11 p.m. evening shift and for our 11 p.m. to 7 a.m. overnight awake shift. Our resident care positions require no certification but we will train. Candidates must be responsible and interested in encouraging residents to live active, satisfying lives at home and within their community.We are a congenial, easy going home and have a great working environment and offer health benefits. Having own transportation or a reliable ride to and from is ideal for these shifts. All positions are full or part time and may include a portion of the weekend.
