Concern Housing is seeking an ADL Counselor to be responsible for providing life skills training services directly to clients and is considered a resource in that they should be familiar with the skills and tasks necessary to help consumers learn self reliance skills. The primary task is to provide hands on assistance to clients to help them to develop the skills necessary to maintain independence in the supported housing program. Position: ADL Counselor, Supported Housing Schedule: Monday - Friday; 9am - 5pm Location: Medford, NY Salary: $45,000/yr. Duties and Responsibilities: - Adherence to all policies as set forth in Concern’s personnel manual. - Provide education and skill building assistance across multiple activities of daily living including but not limited to maintaining personal hygiene, maintenance of the living environment, laundering of clothing, travel training, money management, food preparation, nutrition, shopping, basic self preservation skills, communication skills, community integration, and wellness self- management. - Provide individual counseling and support services as it pertains to life skills development. - Work as an integral part of the case management team. Maintain open communication with the case manager regarding skill assessment and goal planning activities. - Develop training tools and techniques in coordination with case manager to address identified client needs. - Maintain all documentation pertaining to visits and activities as consistent with agency policy. - Conduct site visits as needed and directed by Supervisor. - Attendance at weekly supervision and staff meetings. - Remain current with all Agency required compliance documents and trainings. - Other duties as assigned. Requirements: Three years experience working in a similar setting/and or working with people with mental illness. Bachelor’s degree in Psychology, Rehabilitation, Social Work or other related mental health discipline preferred. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
