Salary: $85,000.00 - $95,000.00 DOE POSITION PURPOSE: We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team. The ideal candidate will play a pivotal role in providing high-quality therapeutic services to individuals facing emotional and psychological challenges. As a Licensed Clinical Social Worker, you will assess clients' needs, develop tailored treatment plans, and implement evidence-based interventions to support their mental health. You will collaborate closely with multidisciplinary teams, including psychologists and psychiatrists, to ensure a holistic approach to patient care. Position Duties: • Conduct comprehensive assessments of clients’ mental health needs. • Develop and implement individualized treatment plans based on client assessments. • Provide individual, family, and group therapy sessions as needed. • Monitor and evaluate clients' progress and treatment efficacy on a regular basis. • Collaborate with other healthcare professionals to ensure an integrated approach to treatment. • Maintain accurate and confidential client records following applicable regulations and standards. • Provide crisis intervention and support to clients in immediate need. QUALIFICATIONS: • Master’s degree in Social Work from an accredited institution. • Current state licensure as a Licensed Clinical Social Worker (LCSW). • A minimum of two years of experience in a clinical mental health setting. • Strong understanding of mental health theories and treatment modalities. • Excellent interpersonal and communication skills, both verbal and written. • Ability to build rapport and trust with clients from diverse backgrounds. • Commitment to ethical practice and ongoing professional development. I try to be as independent as possible - but mobility issues require use a power wheelchair 24/7. My arms are getting weaker and weaker to where reaching/grabbing anything is difficult. I have an accessible van - but no longer drive - so having a valid driver's license is required. Experience would be great but not required. More interested in finding someone I can feel comfortable with/good fit. We can both learn as we go. Very easy going guy - just losing ability to do things myself. Must be motivated, work independently (see something that needs to be picked up, cleaned, put away and just do it) and as part of a care team. Reliability is most important - can pick hours but need to know will work them. Daytime or evening shifts 4 to 8 hours long. You can pick start time - very flexible. Assistance Needed: - Getting into/out of bed, dressed, etc - Meal prep - breakfast, lunch, coffee etc. - Feeding dogs and cleaning any messes - Loading/unloading the dishwasher, lite cleaning/ most just straightening up - Driving me in my accessible van to store/Doc appt., etc. - Helping me get changed/incontinence (in/out of bed with or without Hayer lift) - General help around the house - Lite yard work, watering flowers. - Grocery shopping; dinner prep/clean up Must love dogs - I do dog sitting so I often have several dogs over. Welcome to bring yours to work. Will need transportation - about 8 miles from Downtown Ithaca. Unfortunately, not near bus route - so must have a car. $20 - $22/hour via Consumer Directed Personal Assistance Program (CDPAP) and local Homecare provider company which also includes paid sick leave. Meals included - make lunch for me and yourself. If out running errands - I would buy lumch. Will need to get a general physical and TB test - cost is reimbursed by Homecare company. Very relaxed working environment - I am very easy going, just need reliable help. Everything else we can figure out as we go. QIDP-Case Manager (Santa Fe, NM) Santa Maria El Mirador is seeking a Case Manager/Qualified Intellectual Disabilities Professional. The QIDP carries a caseload of not more than 20 intellectually disabled participants living in the group home setting and is responsible for providing access to appropriate services for these individuals and their families/guardians requiring assistance. The QIDP also assesses situations, sets goals, and obtains the required services as necessary for implementation and monitoring of the quality of program services on an ongoing basis. The QIDP is responsible for maintaining the Individual Program Plan and completing appropriate monthly, quarterly, and annual reports for each participant on their caseload. The QIDP/CM designs program objectives for the purpose of assisting in actual implementation of the person-centered vision. The QIDP monitors programs and trains direct care staff to assure quality (daily, in some situations). The QIDP/CM provides accurate and timely documentation of this process following all ICF/IID regulations and standards. 1. Bachelor’s degree required in a human services field (including, but not limited to social work, sociology, education, psychology or related field). 2. Minimum one year of experience working with individuals with developmental disabilities required. 3. Knowledge of ICF/IID regulations, active treatment, and professional standards of care helpful. 4.
