Referral Bonuses are also available! Concern Housing is seeking a Program Supervisor to be responsible for the overall supervision and management of supervised sites. He/she is responsible for managing the site/residence in a manner, which promotes client growth, safety, and the effective delivery of rehabilitation services. Position: Program Supervisor Location: Port Jefferson, NY (Back House) Schedule: Monday - Friday; 8am - 4pm Salary: $65,000/yr. Duties and Responsibilities: - Assist with the supervision and coordination of all direct care services. - Supervise assigned staff, training of new staff, supervision and training of all program staff. - Ensures that site/residence is in compliance with all regulatory guidelines regarding client care, safety and delivery of services. - Assist with insuring proper documentation for all Medicaid billable services. - Assists with the responsibility of maintaining client charts, specifically documentation which is required such as service plans and reviews, functional assessments, discharge plans, progress notes etc. - Responsible for closing out client charts upon discharge and ensuring that all documentation is complete. - Acts as liaison with other service providers as well as with community agencies such fire and police departments. - Assists with the coordination of house, client and staff schedules, including client’s day activities, staff work schedules, per diem staff, evening and weekend recreation, house meetings etc. - Perform periodic staff evaluations; participation in hiring and dismissing of staff. - Participation in regular supervision meetings with the coordinator. Entitlements. - Participate in program development as well as other agency wide administrative planning meetings. Such as CARF, staff development, health and safety committees. - Maintains Site vehicles. - Responsible for sending and receiving up to date Physicians Authorizations. - On-call coverage. - Remain current with all Agency required compliance documents and trainings. Requirements: Masters Degree in Psychology, Social Work or related mental health field with at least 3 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good leadership and supervisory skills, good verbal and written communication skills and the ability to make sound judgments regarding client care. About the Company: The Arc in Hawaii is a non-profit organization with 70 years of history in Hawaii. We provide both residential and day services for adults with intellectual and developmental disabilities (IDD). Our mission is to support the inclusion and participation of individuals with IDD in their respective communities throughout their lifetime. Role: We are looking for a Home Manager for our domiciliary residential home in the Punchbowl area. The manager will live on-site and have their own quarters within the home. Responsibilities include training, educating, and supervising direct support staff. The Home Manager is charged with ensuring that residents have a safe, protective, and normal environment that allows for growth and development, privacy and confidentiality, and a sense of belonging and family. Job Type: Full-time, exempt Work Schedule: Must be available to work a flexible work shift, including weekends, evenings, overnights, and holidays. Must be able to respond to emergency calls on a 24-hour, 7-day per week basis.
