Grace Community Care of NJ is looking to hire (2) 1:1 Direct Support Professionals (DSP) one available for Day Shift 9am-3pm and another shift 4pm-8pm or Weekends Schedule: Monday- Friday Weekday Shift or Weekends Weekend shift days between Sat or Sun 9 -2pm This Requires Accompanying and transporting 1:1 client to various community engagement outings. Must have experience working with clients with Developmental Disabilities. Must have Driver's License and reliable transportation. CPR Training and Certification in Last 2 Years Must have Access to Wi-Fi/internet/Tablet or Computer for communication and resource access. Proficient in note-writing and time recording. Team player mentality, professionalism, and punctuality. Qualifications: High school diploma or equivalent (Bachelor's degree in related field preferred). Experience in Direct support. Ability to pass Employment and Background checks. Responsibilities Provide direct support to individuals with developmental disabilities in daily living activities, including personal care and assistance with ADLs either in the community such as community centers or their own personal home. Observe and document patient behavior and progress, ensuring their safety and well-being. Collaborate with team members and families to implement care plans. Utilize first aid skills as necessary and maintain a valid driver's license for transportation needs. 1:1 Personal Care Experience Preferred Non-group home role. This is a 1:1 shift. Job Title: Long Term Services and Supports (LTSS) Coordinator Reports to: Long Term Services and Supports (LTSS) Program Manager Location: BOSTON, MA The Boston Center for Independent Living is a frontline civil rights organization led by people with disabilities that advocates to eliminate discrimination, isolation, and segregation by providing advocacy, information and referral, peer support, skills training, PCA services, and transitional services in order to enhance the independence of people with disabilities. Position Overview: The Long Term Services and Supports Coordinator coordinates long-term services and supports (LTSS) for individual BCIL consumers (clients) enrolled in One Care, an innovative and nationally recognized health care program for people with disabilities. LTSS enhances the independence, health, and wellbeing of persons with disabilities. Essential Duties and Responsibilities: • Assesses the long-term services and supports needs of consumers in One Care, advocating for them, and coordinating services. • Providing education on LTSS and independent living to the consumer and their care team with the goal to improve or maintain consumers’ health, independence, and wellbeing in the community. • Builds positive, productive relationships with consumers, whose input is critical to the development of effective LTSS plans. • Arranges and coordinates the authorization and provision of appropriate community long term supports and services and resources, including, but not exclusively, nutrition, personal care attendant services, assistive technology, homemaking services, and chore services. • Builds positive, productive relationships with service providers and One Care plans’ care teams. • Works with LTSS Coordination Program team members and other BCIL staff as needed in service coordination efforts. • Utilizes database systems for BCIL and LTSS coordination • Performs other duties as assigned by the BCIL supervisor.
