Eminence Home Care CT is looking for dedicated and compassionate healthcare professionals to fill the role of Home Care LPN or RN. In this position, you will provide high-quality, patient-centered care to clients in their own homes, promoting their comfort and independence. As part of our dynamic team, you will work collaboratively with other healthcare providers, families, and clients to develop and implement individualized care plans that meet each client's unique needs. Responsibilities Administer medications and treatments as prescribed by the physician. Perform nursing assessments, monitoring vital signs and client condition. Assist clients with daily living activities, ensuring their dignity is upheld. Educate clients and families on health management and care practices. Document patient care and progress accurately and timely. Coordinate with healthcare providers to optimize care delivery. Provide compassionate support and companionship to clients. Requirements Current and valid LPN or RN license in the state of Connecticut. Previous experience in home care, nursing, or a related field preferred. Strong clinical skills with the ability to assess and respond to patient needs independently. Excellent communication and interpersonal skills to effectively interact with clients and families. Ability to work as part of a collaborative healthcare team. Reliable transportation and a valid driver's license. Ability to perform physically demanding tasks and assist clients with mobility. CPR certification required. Activity Therapist PRN
Job Summary The Activity Therapist, serving as a member of the interdisciplinary treatment team, is responsible for providing therapeutic recreational and activity services to adult, adolescent or child patients under DBHH care for emotional or substance abuse problems. The Activity Therapist will help to develop and implement therapeutic treatment plans demonstrating sound application of AT Theory and methodology and therapy sessions for individuals, families or groups. Reports to community and/or state agencies regarding any patient issues as required by law. Represents hospital in legal proceedings as required. Complies with facility safety, infection control and security program. Supports facility internal and external customer service standards. Requirements Education - Bachelor’s degree in a relevant field (e.g., recreational, music, art therapy or occupational therapy) preferred. Healthcare and inpatient psychiatric experience preferred. Teaching or training experience a plus. Required Licenses – None.Certification or registration preferred. Additional Requirements - Must obtain and maintain approved CPR certification as well as successful completion of Management of Agressive Behavior training including restraint and seclusion policies prior to assuming patient care responsibility. Knowledge, Skills & Abilities: Excellent communication, assessment, evaluation, and interpretation skills. Knowledge of psychiatric and addictive disease diagnoses and treatment; flexibility; teaching or training skills helpful; understanding of, or ability to understand, DSMIIIR methodology. Advanced knowledge and clinical skills in diagnosis and treatment of mental healtho Knowledgeable of patient rights and laws pertaining to mental health. Must have basic PC skills that include a combination of working in a Windows Operating System and Microsoft Outlook, Word and Excel and the ability to type a minimum of 30 words per minute. Must have excellent communication skills and be a good listener with a pleasant demeanor that encourages and engages others. Must be able to multi-task, prioritize with strong time management skills. Screening: Must successfully pass background check, drug screen, physical and be able to provide positive employment references.
