RN Home Care Assessment Nurse (PRN)

Eminence Home Care is seeking a dedicated and compassionate PRN RN Assessment Nurse to join our dynamic healthcare team. Responsibilities Conduct initial and ongoing patient assessments to evaluate health status and care needs. Develop individualized care plans in collaboration with healthcare team members and patients. Communicate assessment findings effectively to ensure continuity of care. Monitor patient progress and modify care plans as necessary based on clinical evaluations. Educate patients and their families about care procedures, medication management, and health maintenance. Provide guidance and support to caregivers and families to enhance patient care. Document all assessments, interventions, and outcomes in compliance with regulatory standards. Requirements Current RN license in the state of NH Bachelor's degree in nursing (BSN) preferred. Minimum of 2 years' nursing experience, preferably in home health or community settings. Drivers License and vehicle Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills for effective patient interaction. Ability to work independently and manage time effectively. Current CPR and Basic Life Support (BLS) certification.
