Adult/Family Nurse Practitioner - Waterloo, NY (#TE1045) Location: Waterloo, NY Employment Type: Full-time Salary: $115,000 - $135,000/year (experience dependent) Position Overview Serve in a dynamic healthcare environment focused on comprehensive patient care. You’ll manage acute/chronic conditions, lead care coordination, and drive quality improvements for diverse patient populations. Key Responsibilities Patient Assessment and Diagnosis: Conducting comprehensive assessments of residents' health status, diagnosing acute and chronic conditions, and developing individualized care plans. Medication Management: Prescribing, administering, and monitoring medications, ensuring appropriate dosages and addressing potential side effects. This is a significant responsibility for NPs. Chronic Disease Management: Managing common chronic conditions prevalent in elderly populations, such as diabetes, hypertension, heart disease, and dementia. Acute Care: Providing care for acute illnesses and injuries, such as infections, falls, and exacerbations of chronic conditions, often in collaboration with physicians. Collaboration and Communication: Working closely with physicians, nurses, therapists, social workers, and families to coordinate care and ensure optimal patient outcomes. Communication with families about patient status and care plans is crucial. Patient Education: Educating residents and their families about health conditions, treatment options, and preventative care strategies. Documentation: Maintaining accurate and detailed medical records, documenting all patient encounters, assessments, and interventions. This is essential for legal and quality of care purposes. Quality Improvement: Participating in quality improvement initiatives to enhance the delivery of care and patient safety within the facility. Requirements Qualifications Education: Master’s degree in Nursing (FNP/AGNP specialization) Licensure: Active NYS NP license (or eligibility to obtain) Certification: Board Certified/Eligible (FNP, AGNP, or related specialty) Experience: Clinical experience in adult/geriatric care (preferred) Background in chronic disease management (e.g., diabetes, dementia) Technical Skills: Patient assessment, diagnosis, and care planning Medication management and EHR documentation Soft Skills: Strong interdisciplinary collaboration Patient/family education expertise Attention to detail in clinical documentation Benefits Why Join Us?
