PER-DIEM | 8-HR SHIFT | WEEKEND | AM SHIFT What We Do: Charter Oak Hospital offers compassionate and individualized treatment for mental health and substance use disorders. What You’ll Do: Our CD Counselor make a difference in the lives of our patients by providing quality care to individuals with substance abuse or chemical dependency problems. Our dedicated professionals work tirelessly to create an environment that fosters healing, growth, and long-term wellness. Incumbent responsibilities may include: • Assess patients' stage and extent of chemical use • Conducts didactic and other appropriate patient groups • Guide patients in meeting treatment goals • Develops and implements a plan of treatment, in conjunction with members of multi-disciplinary team Join us in making a meaningful impact in the lives of those we serve!
