Riccobene Associates Family Dentistry is looking for a part time Registered Dental Hygienist to join our Southport (Bolivia, NC) office! This position would be part time Mondays and Fridays. DENTAL HYGIENIST POSITION SUMMARY A Dental Hygienist for Riccobene Associates Family Dentistry works together with our dentists to meet the oral health needs of our patients. A Dental Hygienist is responsible for promoting dental health by completing dental prophylaxis, providing oral cancer screenings, periodontal maintenance, charting dental solutions, willingness to assist with a variety of responsibilities in the dental office, and performing in compliance with Riccobene Associates Family Dentistry’s highest standard of care. DENTAL HYGIENIST KEY COMPETENCIES Assess dental condition and needs of patient using patient screening procedures, including medical history review, dental charting, and perio charting Delivers direct patient care using established dental hygienist procedures Takes patient vital signs Performs routine treatment procedures, such as cleaning and polishing Takes radiographs required for diagnosis of treatment needs by the dentist Applies sealants and fluorides Electronic record keeping Documents patient dental history and chief complaint Records and reports pertinent observations Teaches patients how to prevent tooth decay and gum disease through proper diet and oral home care Cleans and sterilizes instruments Completes Treatment Plan as prescribed by the dentist Reviews and explains treatment plan and associates fees with patient Ability to articulate and express patients concerns to dentist Primary treatment coordinator Ancillary duties as needed Requirements Must be present by 7:45am as clinic opens at 8am (operational hours may vary based on location); individual must be reliable. Role: POSITION IS 32 or 40 HOURS WEEKLY The Social Worker plans, coordinates and manages social work services for frail older adults and caregivers of Elder Service Plan (ESP) within the community as part of Integrated Disciplinary Team (IDT). Duties include but are not limited to conducting initial, semi, and annual assessments; participating in the Intake process for potential participants; providing supportive counseling to ESP participants and their family members; actively participates in Interdisciplinary Plan of Care meetings and morning meetings; arranges family meetings for IDT. Responsibilities: • Participates in the intake screening process in potential participants residences, conducts a bio-psychosocial assessment. • Conducts timely initial, annual, and semiannual psychosocial assessments in client's residences in order to develop participants plan of care to include assessment tools of PHQ-9, MOCA, and CDR. • Actively participates in required meetings to include but not limited to Morning/Weekly Team, Plan of Care, and Family Meetings. • Responsible for attending monthly service plan meetings for participants in assisted living and nursing home facilities. • Provides individual and family supportive counseling at ESP and client's residences and conducts participant and caregiver support groups. • Manages clients during psychiatric hospitalizations. • Assists/advocates for interdisciplinary team, clients and families with community resources. • Works with financial staff to assist with bi-annual financial redeterminations. • Must be able to travel between Harbor sites and participant's residences as needed in a timely manner. Requirements: • MSW from an accredited school of social work, LCSW or LICSW required, current BLS Certification required • 1 year of experience working with the frail elder population (paid or volunteer experience). • 2 years of professional experience of which 1 year was in providing direct behavioral health therapy to an adult population. • Basic skills for Microsoft Office applications: Word, Excel and Outlook. • 2 or more years of geriatric experience preferred. • Experience with dementia care, substance abuse and end of life care preferred. • Critical thinking skills to problem solve individually and in a team setting • Bilingual a plus; English/Spanish or English/Haitian Creole. POSITION SUMMARY The primary role of the Patient Coordinator is to help the dental office stay on schedule, schedules new appointments, enters patient data for billing purposes, communicates with patients in an efficient and empathetic manner, and generally keeps the office stocked with necessary supplies.
