Join Our Team   CER is a locally operated, private, and publicly funded 501(c)(3) nonprofit agency based in Honolulu, HI.  CER is dedicated to provides inclusive care to people who are diagnosed with substance abuse, mental illness, and homelessness.  CER promotes independence and stability and aims to assist clients and provide the utmost care through our:  Mission:  To provide progressive and inclusive health care, empowering individuals to lead fuller lives.  Vision:  All individuals are accorded full rights and status as members of the community.  The purpose of this position is to provide Stabilization Intensive Case Management services and community-based beds for individuals experiencing emotional, behavioral, or psychological crises and who may be homeless, and/or medically vulnerable in a safe therapeutic environment. The program provides adults, eighteen (18) years and older a structured setting for a short-term stay for up to fourteen (14) days. The service components included in this program are intensive case management and care coordination, peer support, bed stabilization, and stabilization case management program funds.    You will be developing clinically based treatment relationships with persons diagnosed with serious and persistent mental illness and helping them integrate into the community using a structured recovery plan: obtaining psychiatrist visits, medications, housing, substance abuse treatment, legal coordination and other services. You will report to the crisis stabilization facility in Kaneohe to work with the mental health clients, work with the treatment team, communicate with stakeholders in the community, and complete vital documentation. Case Mangers also host skill building and recreational groups onsite to aide in client recovery. **We are Currently Hiring for Swing/Evening Shifts** Evening Shift Hours: 3pm-11pm Full time: Four or five 8-hour shifts (32-40 hours per week) OR Part time: Two or Three 8-hour shifts (16-24 hours per week) Requirements Required High School Diploma/GED or equivalent. Preferred Bachelor’s degree in Psychology, Social Work, Health, Nursing, Counseling (or similar degree with course work in psychology). Preferred 6-12 months supervised experience (work, volunteer, or a combination) with at least one of the following populations: severely mentally ill (SMI/SPMI), developmentally disabled (DD), elderly, children, or other special needs populations. Preferred: Access to a vehicle and valid driver’s license, clean driving abstract, and valid safety check. We do have non-driving positions available but paid at a lower rate of pay. People who do well in this role are passionate about helping people, is skilled in communication, time management, and are able to respond calmly under pressure/during crisis situations. Benefits $18 - $20 per hour, depending on background, education, and performance. Plus monetary benefits listed below. Integrated Housing Stability Specialist
Access Services is seeking a compassionate and resourceful Integrated Housing Stability Specialist in Fort Washington, PA to empower individuals in Montgomery County, PA on their journey toward lasting stability and wellness. This full time, dual-role position blends Housing Stability support with Mobile Psychiatric Rehabilitation services, offering holistic care to individuals enrolled in both the Starting Point and Permanent Supportive Housing (PSH) programs. The Integrated Housing Stability Specialist will provide a patient supportive presence, be flexible, and able to be an advocate and liaison for those they serve. The Integrated Housing Stability Specialist will be motivated by bringing meaningful life change to those they serve.  Their ability to organize, coordinate, and bridge gaps between individuals, landlords, and community resources will offer personalized support to help each individual grow and flourish in their housing and mental health journey.   Essential Functions of Housing Stability:  Meet with referred individuals along with the Housing Locator, and with individuals moving into housing with assistances from Access Services.  Support individuals in developing Housing Stability Plans and updating it together every three months.  Maintain organized documentation for move-in and recertification requirements, and ongoing documentation for all individuals served.  Conduct monthly (or more frequent) home visits and support during transitions.  Support with transportation for essential health appointments, job interviews, and activities that contribute to housing stability.  Identify and address barriers to housing stability with creative solutions.  Connect individuals to social, health, and benefit resources.  Ensure compliance with HUD requirements and funding guidelines.  Participate in county, committee, and consumer meetings and case reviews.   Essential Functions of Mobile Psychiatric Rehabilitation:  Develop and regularly update Individual Rehabilitation Plans (IRPs) that are recovery-oriented, strengths based, and person-centered with the individual served and the team.  Collaborate with individuals in the program to develop strategies for identifying, achieving and maintaining their personal goals.   Offer help and support respectfully and with a person-centered approach.
