The facilitator will provide service recipients high-quality community integration training, volunteer opportunities, and employment development and exploration. Conducts client contacts, schedules, and weekly activities consistent with the client's Individual Service Plan (ISP) long- and short-term goals. A facilitator will also assist the client and other planning team members with the intake and assessment process if the Program requires it. Depending on the physical needs of clients, some self-care assistance, such as toileting and feeding, may be necessary to support the client's success. Client Facilitator reports to assistant director. Client Facilitator supervises no other staff in the facility. Minimum qualifications: Must be at least 18 years of age. Must pass Criminal Record Clearance (DOJ and FBI Background check) High school diploma or at least one year of experience working with individuals with developmental disabilities.
